The Department focuses on newborn screening tests. It carries out and coordinates the tests conducted in eight centres around Poland. The Department's lab conducts screening for almost 100,000 newborns (26% of the population). It offers a wide range of tests in clinical chemistry, covering over 60 parameters, e.g. determining the levels of more than 30 hormones, vitamin concentration, tumour markers and bone turnover markers, as well as a number of parameters for diagnosis of rare diseases, such as galactosaemia, phenylketonuria, fatty-acid oxidation disorders, organic acidemias, aminoacidurias and others.

The Department conducts scientific research on screening tests, diagnosis of rare metabolism disorders, biochemistry of bone turnover and certain biochemical parameters in pregnant women, including the analysis of the impact of tobacco smoking. Moreover, under the Iodine Deficiency Elimination Programme financed by the Polish Ministry of Health, we monitor iodine supplementation in pregnant women and iodine deficiency in newborns.

The Department performs tests for the IMC and for outpatient clinics and hospitals around Poland.
Main areas of the Department's clinical and scientific activities:

• diagnostic imaging of solid tumours at the developmental age, especially tumours originating from the muscucentraloskeletal system: bones and soft bodily parts. We work for the Department of Surgical Oncology at the IMC, the largest of this kind in Poland;
• diagnosis of lesions in lungs and gastrointestinal tract in the course of cystic fibrosis.
• We work for a national reference for CF, located at the Paediatrics Clinic at the IMC.
• prenatal diagnosis using MRI. As one of the few diagnostic centres in Poland, we have performed MRI tests for pregnant women with suspected defects in the foetus. Such tests have been performed for several years.
• We also diagnose diseases in pregnant women.
• We perform prenatal testing starting from the 20th week of pregnancy for all centres in Poland.
• Radiological diagnosis of the craniofacial area. We perform it for the reference centre, i.e. the Department of Surgery of Craniofacial Defects at the Surgery Clinic for Children and Adolescents at the IMC;
• diagnosis of congenital malformations;
• diagnosis of central nervous system (CNS) disorders in children.

Scientific research activities:

The Medical Genetics Department is a national reference centre for genetic counselling, testing and diagnostics of many genetically determined diseases.

The Medical Genetics Department offers complex diagnostic (including prenatal) and scientific testing based on state-of-the-art cytogenetic and molecular methods as well as medical consultation provided by experienced doctors – clinical geneticists in the genetic counselling clinic.

Our scientific research and diagnostic testing comply with global standards. The Medical Genetics Department is certified by international external quality assessment schemes for diagnostic testing (CEQA, Labquality, EMQN and CF-Network among others).

The Medical Genetics Department conducts studies on chromosomal aberrations and molecular pathology of several dozen diseases and its diagnostic offering is constantly widened. In many cases the diagnostic tests offered by the Medical Genetics Department are unique in Poland. The Medical Genetics Department was the first institution in Poland to introduce the Microarray-based Comparative Genomic Hybridisation (aCGH) method to diagnostics. Currently the test based on this technique is one of the basic diagnostic tests conducted in Poland and worldwide. Over 73000 patients have been testing in the Medical Genetics Department using this method so far.

The Medical Genetics Department implements research projects financed among others by the Ministry of Science and Higher Education (the State Committee for Scientific Research, the National Science Centre), the Foundation for Polish Science and the European Funds. Over 80 research projects have been conducted in the Medical Genetics Department so far.

Only up to 2006 the employees of the Medical Genetics Department had published over 150 papers in prestigious national and foreign journals, issued 5 monographs and prepared, at the request of the Ministry of Health, 10 expert reports on diagnostics of genetically determined diseases.

The employees of the Medical Genetics Department are members of international research consortia and numerous national and international scientific societies. They are also winners of numerous national and international awards and honourable mentions.

The Medical Genetics Department cooperates with many medical genetics centres both in Poland and abroad, among others with the prestigious research and diagnostic centre Baylor College of Medicine w Houston in the United States and KU Leuven in Belgium.

The Medical Genetics Department is a reference centre for specialist training in clinical genetics for doctors and in laboratory medical genetics for laboratory analysts recognised by the Ministry of Health. Its employees co-created the programme for specialisation in laboratory medical genetics.

The Medical Genetics Department offers student internships and courses as well as individual training human genetics, cytogenetics and molecular biology.

The department has state-of-the-art testing and laboratory equipment (among others ABI capillary sequencer, MiSeq next-generation sequencing system, microarray scanners, real-time PCR system, Fluoview microscope system with intravital microscope testing module, fluorescence microscopes and computer-assisted karyotyping system coupled with microscopes as well as laminar flow cabinets and CO2 incubators to grow and maintain cell cultures).

Our scientific activities are closely linked with our clinical activities and comprise the following:

allergic disease in its various forms (atopic dermatitis, bronchial asthma, seasonal allergic rhinitis and conjunctivitis, recurrent inflammation of the airways and ears), with particular emphasis on concurrent and early diagnosis of food allergies); congenital and acquired immunodeficiency in children.

 

Cardiology Department

The Cardiology Department opened on 1 January 2013. Its activities focus on non-invasive assessment of the cardiovascular system – the Department performs foetal echocardiography, cardiac examination of infants, including premature babies, examinations of children and adolescents treated with anti-cancer drugs, and pregnant women. In 2013, the Department launched a Cardiology Clinic for Pregnant Women.

The Department conducts its scientific research based on the results of clinical trials. In addition to clinical topics, the Department's staff are interested primarily in pathomechanisms of cardiovascular complications in pregnancy, including hypertension in pregnancy.

Under internal grants and cooperation with other IMC units, the Department currently undertakes research into the following topics:

• The use of modern methods of non-invasive assessment of late cardiotoxicity of complex anti-cancer therapy of Ewing's sarcoma in children and adolescents.
• Evaluation of the microRNA profile in various forms of clinical dysfunctions or overloading of the left ventricle in pregnant women – a pilot study, co-operation with the Medical University of Warsaw.
• Markers of cardiac overload and myocardial fibrosis in pregnancy.
• Evaluation of pregnancies in patients diagnosed with suspected congenital heart defects in foetuses in a prenatal cardiac examination.
• Analysis of indications to perform echocardiography in newborns, especially in premature babies – based on own materials.

Projects financed by the National Science Centre (NCN):

• No. 2013/11/N/NZ5/03388 – microRNA associated with hypertrophy and fibrosis of the myocardium in the molecular mechanisms of response in the left ventricle to volume overload and pressure overload in pregnant women.

Project financed from other sources:

Cystic Fibrosis Center

Pursuant to the Organisational Rules of the Institute of Mother and Child (§55), the tasks of the Cystic Fibrosis Center include, in particular:

1)  diagnosis and treatment of lung diseases in children, including cystic fibrosis. The Center is accredited for running a specialisation in children's lung diseases.

2)  scientific research, clinical trials and R&D in the field of cystic fibrosis in children and adolescents.

Re. 1.

The Institute of Mother and Child (IMC) is the largest paediatric centre in Poland handling CF patients aged up to 18. More than 370 children with cystic fibrosis are currently in our care. This represents approx. 40% of the population of children with this disease in Poland.

The Center offers diagnosis and treatment through its Cystic Fibrosis Clinic as well as the Allergology, Cystic Fibrosis and Lung Disease Unit.

Our long-standing Sweat Test Lab provides services for patients from all over Poland, acting as a referral centre.

We conduct infant screening for cystic fibrosis, offering diagnostic tests, diagnosis and further specialised health care.

We offer multi-speciality care for cystic fibrosis patients, including outpatient treatment as well as hospitalisation in periods of exacerbated bronchopulmonary symptoms.

We offer pre-qualification and refer patients at the end stage of the bronchopulmonary disease to transplantation centres. We prepare patients for lung transplantation and offer follow-up health care.

Our Cystic Fibrosis Center consistently takes steps to establish a model referral centre for cystic fibrosis patients (a cystic fibrosis centre) in accordance with the current international standards.

Employees of the Center are actively involved in developing a multi-speciality care model for cystic fibrosis patients on the basis of its specialised unit as well as the Cystic Fibrosis Clinic. As cystic fibrosis affects multiple bodily organs and systems, patients need comprehensive and multi-speciality treatment including prevention and treatment of bronchopulmonary disease, exocrine pancreatic insufficiency therapy, nutritional therapy and treatment of CF complications. Apart from doctors and nurses, a nutritionist, a psychologist and a physiotherapist are permanently involved in treating CF patients. An individual treatment strategy for each patient is agreed at regular meetings of the multidisciplinary team.

Comprehensive outpatient care of CF patients helps to:

• improve the effectiveness of CF treatment based on currently applicable standards,
• reduce the number of hospitalisations caused by cystic fibrosis and its complications,
• extend the lifespan of CF patients.

Re. 2:

A) Current research topics:

– The impact of nutritional factors on bone mineral density in adolescents with cystic fibrosis.

– Epidemiology of Pseudomonas aeruginosa infections among children with cystic fibrosis.

– Quality of life and occurrence of depression and anxiety symptoms among Polish CF patients and their caregivers: An international comparative study.

Two European projects have been completed, more are under preparation.

B) R&D tasks:

Participation in international multi-centre clinical trials on innovative drugs (at present, new inhaled antibiotics and pancreatic enzymes are under study).

The Center has many years of experience in clinical trials related to CF, with over 20 completed trials, which means that our patients have had access to state-of-the-art therapies.

 

 

Pathomorphology Department

The Pathomorphology Department has co-operated for many years with the Oncological Surgery Clinic for Children and Adolescents at the IMC. Our plans and scientific endeavours also focus on children's oncology. We conduct in-depth studies on the morphology of solid tumours in children and adolescents, with special emphasis on musculo-skeletal cancer (tumours of periosteal soft tissues and bones). These studies are focused on searching for morphological prognostic markers, with particular emphasis of preoperative chemotherapy used in the treatment of these tumours, with the application of immunohistochemical diagnostic methods and molecular tests.

The Molecular Oncology Laboratory, which has been part of the Department since late 2001, began molecular tests in solid tumours in children, with particular emphasis on chromosomal abnormalities occurring in those tumours. The Laboratory also studies the expression of tumour suppressor genes and oncogenes involved in the formation and development of tumours.

Within its scientific activity, the Department has participated in research projects conducted in collaboration with the IMC Obstetrics and Gynaecology Clinic. The topics in focus included occurrence of infections of the lower genital tract in women in correlation with the occurrence of preterm labour as well as clinical and morphological problems connected with labour from complicated pregnancy with preeclampsia.

In our research projects related to childhood cancers, we deal with important life processes in cancer cells (proliferation, apoptosis, angiogenesis, drug resistance), in small round cell tumours of soft periosteal tissues and bones, as well as osteosarcomas, running an analysis at the level of immunohistochemical tests as well as molecular tests. The combination of both diagnostic methods takes place in our ongoing research projects conducted in collaboration with the Paediatric Surgical Oncology Clinic at the IMC.

For many years, the Pathomorphology Department has provided consultations for the Polish Paediatric Group for the Treatment of Solid Tumours in Children in areas such as neuroblastoma, nephroblastoma, bone tumours and solid tumours of soft periosteal tissues.

In 2007, as a result of many years of collaboration, the competitions committee awarded a task to the Department under the National Programme for Against Cancer.

In total, in the last two years, the Department's staff members took part in work on two topics under the so-called ordered projects financed by the Committee for Scientific Research, four research projects and two doctoral grant projects. The Department has also prepared expert opinions for the Polish Ministry of Health.

Apart from the aforementioned research and development activities, our Department also offers services for the IMC inpatient and outpatient clinics (based on its statutes) as well as commercial activities.

For five years now, the Pathomorphology Department has participated in the Cervical Cancer Prevention Programme under a contract with the National Health Fund (NFZ).

Rehabilitation Department

The Rehabilitation Department was founded in 1983. During its first years, it primarily performed pioneering work and research related to the rehabilitation in paediatric oncology. During this period, the IMC staff developed the foundations for the assessment of physical performance and fitness of children undergoing oncological treatment. Also, the main directions of rehabilitations as well as accompanying verification methods were developed. The second stream of work was devoted to physical therapy of children suffering from cystic fibrosis. The Department was the first one in Poland to introduce the latest physiotherapy methods used in Europe in the treatment of cystic fibrosis patients. In line with the IMC policy, the Department focuses on the rehabilitation of the youngest patients, i.e. newborns and infants, especially ones with neurodevelopmental disorders. With this in mind, the Department maintains specialist staff who have undergone relevant professional training. Since 2009, the Physical Therapy and Kinesiotherapy Laboratory has operated in a new facility, with state-of-the-art equipment for physical therapy, also adapted for adult patients.

The activities of the Department focus on mental health (esp. infant mental health), perinatal psychology, clinical psychology of children and families, and early intervention.

Scientific activities

The scientific activity conducted by the Department is focused around the psychological impact of premature birth for the development of children and their families as well as the exploration of early psychological mechanisms (associated with the relationship between parents and children) which play a protective role in psychological development. The Department also studies the psychological situation of children born with craniofacial defects and their families.

Examples of projects carried out by the Department:

A multi-annual statutory task conducted in 2006–2009:

• Long-term impact of premature birth on the development of relations between parents and the child.

Statutory task for 2010–2012:

• Fathers and mothers in the perinatal period: the emotional condition of the parents and the perception of the baby.

Statutory tasks for 2012–2014:

• The psychological situation of children with cleft lip and palate and of their families.

Participation in the research project entitled ‘Relational and biological background of self-regulation processes in prematurely born children in the first year of life' (No. N N106 045734) was financed by the Polish Ministry of Science and Higher Education in 2008–2011.

The project was implemented by the Department of Psychology of the University of Warsaw in collaboration with the Institute of Mother and Child (ZWIP, Neonatology and Neonatal Intensive Care Clinic, Screening Tests Department) and the Interdisciplinary Centre for Behavioural Genetic Studies, the Clinical Hospital at the Medical University of Warsaw and the Holy Family Hospital in Warsaw.

The Department engages in continued scientific cooperation with the following IMC units: Neonatology and Neonatal Intensive Care Clinic, Nutrition Department, Orthodontic Clinic and others.

The Nutrition Department is involved in developing nutrition policy on food safety and nutrition of infants, children, adolescents and women during pregnancy and lactation.

The Department:

• conducts scientific research in the area of physiological foundations of nutrition and eating disorders in children and adolescents, including insufficient body weight and simple obesity;
• offers services in the field of medical and nutritional counselling as well as diet counselling;
• carries out assessment and issues opinions on foodstuffs, including foodstuffs for special nutritional and medical uses, as well as water intended for infants and young children;
• engages in educational activities in the field of medical issues, nutrition and diet.

 

Paediatric Surgery Clinic

Our team is composed of paediatric and oral surgeons. IMC Paediatric Surgery Clinic focuses on treatment and research in three key areas:

• general paediatric and neonatal surgery;
• comprehensive treatment of craniofacial defects and developmental abnormalities of face and cranium;
• treatment of congenital malformations of external genitourinary organs.

Our priority in general child surgery and infant surgery is the treatment of congenital and developmental malformations that require surgical intervention directly after birth, in infancy or early childhood.

The clinic has developed diagnostic and treatment standards for congenital defects of digestive system, abdominal walls, nervous system, etc. The neonatal surgery standards include also protocols for handling cases of diagnosed congenital defects (interdisciplinary team for prenatal and neonatal treatment formed in cooperation with the Obstetrics and Gynaecology Clinic).
In cooperation with the Neonatology and Infant Intensive Therapy Clinic, we have developed protocols that are followed in surgical pathologies typical for premature infants with birth weight under 1000 g.

The Clinic also focuses on comprehensive treatment of congenital craniofacial abnormalities, including cleft lip, cleft palate, facial cleft, acrania. Our multi-discipline team of doctors treats patients from early infancy until adulthood.
The Clinic's unique achievement is a single-step treatment method for cleft lip and palate, as well as an innovative treatment method of Pierre Robin Syndrome, following an original paediatric surgery protocol.

Another important area of focus is surgical treatment of giant melanocytic naevi and haemangiomas. We also work with oncological clinics on removal of cancers from cranium and face. The clinic specialises also in the treatment of congenital defects of genitals such as Hypospadias.

 

Department of Oncology and Surgical Oncology for Children and Youth

 The Paediatric Surgical Oncology Clinic is a highly specialised treatment centre, in particular in the treatment of tumours in children, including:

• Coordination and treatment centre for the treatment of bone cancer in children and youth,
• Complete diagnostics and comprehensive treatment (chemotherapy, surgery, radiotherapy) of childhood and youth cancer,
• Complete diagnostics and comprehensive treatment of child patients from all regions of Poland, with cancers of soft tissue, bone, sympathetic nervous system, kidneys, germ cells, thyroid gland, liver; skin abnormalities and other neoplastic and tumour-like processes,

• We are a referral centre for conserving, less radical therapies that make it possible to salvage a child's limb through the use of oncological endoprosthetic solutions and so called growing prosthetics. The prosthetic limb grows. In such cases we use an oncological endoprosthesis that extends as the child grows. Institute of Mother and Child is among the few medical centres globally, where the endoprosthesis is extended through the application of electromagnetic field, making the procedure quick and non-invasive.
• We cooperate with a number of domestic and foreign medical organizations.

Diagnostics and treatment of nervous system diseases are carried out, with particular emphasis on epilepsy, especially drug-resistant epilepsy, non-epileptic seizures, degenerative diseases of the CNS, headaches and neurodevelopmental disorders in children;Badania genetyczne uogólnionych padaczek idiopatycznych. The purpose of these tests is to create the principles of comprehensive diagnosis, treatment and genetic counseling in genetically determined epilepsy.

The clinic conducts research and provides treatment in:

• Inborn errors of metabolism,

• Cystic fibrosis,

• Chronic diseases of respiratory tract,

• Kidney stone disease linked to the metabolic syndrome

• Gastroenterology with particular focus on dietary intolerances and allergies as well as eating disorders.

The IMC Paediatric Clinic is a certified to grant specialisation in paediatric medicine and pulmonology.

Services: inpatient treatment of children aged 0–18, particularly patients with the following:

• Phenylketonuria – Poland's largest and oldest treatment centre (over 40 years), referral clinic for diagnosis of hyperphenylalaninemia and treatment of non-standard forms of phenylketonuria,

• Galactosaemia – referral clinic for comprehensive diagnostics and treatment,

• Diagnostics of rare metabolism defects on the basis of MS/MS screening tests of newborns,

• Hyperlipidaemia and other arteriosclerosis risk factors,

• Cystic fibrosis (CF) – centre for comprehensive diagnosis and treatment established 40 years ago, the clinic provides treatment to the largest number of CF patients registered in Poland,

• Chronic lung diseases, congenital malformations of the respiratory system,

• Stone disease and defects of urinary tract,

• Somatic development disorders.

Pregnancy, childbirth and post-partum disorders, in particular preterm birth problems and intrauterine growth restriction (IUGR);

• Prenatal diagnostic testing,
• Developmental age gynaecology,
• Pregnancy and postpartum care,
• Preconception care,
• Prevention, diagnostics and treatment of tumours/cancer,
• Birth control advice,
• Gynaecological endocrinology,
• Perinatology.

Research activities of the Clinic's doctors focus on maternal – foetal medicine, fertility problems, perinatology and women's diseases. The clinic has a special Interdisciplinary Team for Researching Foetal Defects
