The IMC provides each patient with access to his/her medical records. Medical records can be made available in an ordinary copy, a certified copy or an extract.
You can receive your medical records in person, at the IMC, or you can ask for it to be sent to you by registered mail, upon payment (collect on delivery).
To obtain your records, please submit a relevant application at the IMC Post Room.
You can collect your medical records at the Organisational Department, Lipsk Building, 2nd floor, Room 222, tel. +48 22-32-77-295, 22-32-77-483 or 484. Please remember that in keeping with the current legislation (Act on Patients' Rights and the Ombudsman for Patients' Rights), underage patients after their 16th birthday must agree for their medical records to be passed on to their legal guardians.
Applications to release medical records are processed immediately (within 30 days following the submission of an application). When collecting their records, patients must make a payment at the IMC Cash Desk (Lipsk Building, 1st floor, room 128C). Payments for the provision of medical records are made in accordance with the IMC price list – Chapter XX.I Other services.
