Referral and admission to hospital

The IMC clinics and units admit patients with a referral issued by any doctor, including a private health care physician. No referral is required in life-threatening situations.

We ask patients to report to the Admissions Room.

To be admitted, patients must produce all of the following documents:

  • a valid referral which contains a medical diagnosis and the patient's full personal details (full name, date of birth, PESEL number and address of residence),
  • a medical records book (książeczka zdrowia) with up-to-date information on immunisations,
  • the patient's PESEL number.

Admission of a child begins at the registration desk. A nurse at the Paediatric Admissions room will ask you to produce the documents and will enter the child into the patient register. Then a doctor at the Admissions Room will hold an interview concerning the child's current and future health situation.
Relevant information to be shared with the doctor include any worrying symptoms noticed, allergies, any chronic treatments, recent diseases and immunisations. The doctor will examine the child, take his/her weight and height, and refer the patient to a specific unit of the IMC.

At the Obstetrics and Gynaecology Clinic, patients are registered at the Obstetrics Admissions Room. After an examination, the patient will be referred to a relevant unit.
